REGISTER IN FOUR STEPS
1. Download this form, chose which version you'd like: Word or PDF

2. Fill out the Registration form in one of two ways:
• Complete the form in Adobe Acrobat. Print and mail the signed form with
   payment in full.
• Print out the form. Handwrite your responses and mail the signed form with
   payment in full.

3. Write a check or money order to Ethnomusic, Inc.

4. Mail the signed registration form with the check to Ethnomusic, Inc. to:
